2009 Horizon Grants from the St. Johns County Education Foundation
You may only apply for one classroom Horizon Grant this year. The due date for the 2009 Horizon Grants will be September 18, 2009. This means that the grants must be received in the St. Johns County Education Foundation office on or before September 18th at 5 P.M. No grants will be accepted after this date.
There is a new process for the 2009 Horizon Grants. An application will have three distinct parts. All of these parts (the two printed and the one on-line) must be completed before any application can be considered.
1. Part one is the Grant Cover sheet. This must be downloaded, filled out on the computer, and printed. It must be signed by the teacher and the principal and then sent to Donna Lueders at the School Board.2009-2010 Grant Cover Sheet.doc
2. Part two is the proposed Grant Budget sheet. This must be downloaded, filled out on the computer, and printed. It must be sent to Donna Lueders at the School Board together with the cover sheet. Proposed Budget Form.doc
3. Part three is the Detailed Grant Proposal. THIS MUST BE DONE ON-LINE at <https://surveys.stjohns.k12.fl.us:443/TakeSurvey.aspx?SurveyID=p2MH692> NO PRINTED APPLICATIONS WILL BE CONSIDERED! To help in your preparation for the on-line application, we have given you a copy of the questions that will be asked on-line.
